LOTUS PRIMARY CARE
Direct Primary Care Membership
Your monthly membership fee includes:
30 or 60 minute appointments
Direct access and communication with your doctor
Preventive health and wellness care
Diagnosis and management of most medical conditions
Primary care in-office procedures
Examples: ear irrigation, EKG, bursa/joint/tendon injections, wart removal, skin biopsies, uncomplicated laceration repair
Most in-office tests—including urine, blood, and respiratory illness testing
Tests which need to be sent to outside laboratories will be billed separately—either through Lotus directly, or the lab can bill your insurance
Coordination of care with specialists or hospitals
House calls (on Cape Cod only) for patients who are medically unable to come in, but need to be seen in person
Membership Rates
Single Adult: $250 per month
Couple: $400 per month
Family*: $500 per month
*Includes 2 adults and up to 3 children ages 0 - 25
*Additional children are $50 per month per child
One-Time Registration Fee
$200 per person, max $500 per family
