All year-round and seasonal memberships include:
Direct communication with your personal physician
Unrushed office visits
Preventive health and wellness care
Diagnosis and management of most medical conditions
House calls (Cape Cod only) for patients who are medically homebound
Telehealth availability for patients in MA, FL, NY, ME, or TX
Primary care in-office procedures
Some examples: EKG, earwax removal, certain joint or tendon injections, skin procedures, uncomplicated laceration repair
In-office tests for respiratory illnesses, UTIs, diabetes, and more
Tests that are sent to outside laboratories will be billed separately, and may be covered by your insurance
Access to advanced diagnostic testing at special member rates
Coordination of care with specialists or hospitals

Individual: $250 /month
Couple: $400 /month
Family*: $500 /month
One-Time Registration Fee
$200 /person (max. $500 /family)
*Family memberships include 2 adults and up to 3 children ages 0 - 25. Additional children are $50 /month per child

Seasonal memberships may be renewed each year as capacity allows.
Seasonal memberships may also be extended by converting them to monthly Direct Primary Care memberships. If care remains continuous without gaps, then the registration fee is waived.

Comprehensive Health and Wellness Assessment

60-minute initial visit to discuss your health history and goals
Pre-visit forms are provided, to make the most of your time
A comprehensive panel of blood and urine tests to assess:

An extensive menu of additional advanced testing options, available at member-only discounted rates

60-minute follow-up visit to review your results and create a personalized care plan
Option to convert to any Lotus membership plan within 12 months and receive a $300 account credit
